If you are interested in becoming a foster, kinship, or adoptive caregiver, your journey starts by completing the inquiry form below.  Making an inquiry is the first step to learn more about the process, which may seem overwhelming right now. Rest assured there are people and resources available to help.

The form gives you the option of selecting an agency or speaking to a specialist. Selecting an agency will require a little research to find one near you that you feel would be the best fit.  To choose an agency, start by utilizing the ODJFS Statewide Agency Directory  and locating a few in your area.

If you are not ready to choose an agency, that’s okay. Select “I’m not ready to pick an agency” from the dropdown to speak with a specialist who can answer your questions and provide more information.

The information in this form will be sent directly to the agency you selected or to The Ohio Child Care Resource & Referral Association (OCCRRA) Service Delivery Areas (SDAs) Hubs if an agency is not identified. You will hear back from the agency within 7 business days. Please note when an agency is selected this information will also be shared with The Ohio Child Care Resource & Referral Association (OCCRRA) Service Delivery Areas (SDAs) Hubs for quality assurance purposes.
